Summary of Key Functions

The Case Management Assistant will be responsible for helping in collecting, maintaining, and processing the case of Persons with Specific Needs (PSNs) including mentally or physically challenged individuals, illness, mental health issues or life-altering conditions to gain access to rehabilitation programs and other community services. This position reports to the Case Manager through the Project Coordinator. His/her duty includes arranging meetings with PSNs/PWDs and their families, connecting them with professional services, and maintaining consistent communication to assess their well-being and make changes to their care/service delivery as needed. The candidate will also be expected to establish and maintain contacts with the service providers and communicate with their clients regularly.

 

Roles and Responsibilities

 

  • Identification of individual cases with specific needs and those at risk.
  • Filling, maintaining, and documenting case history reports. Ensuring proper documentation of individualized cases and follow-up processes.
  • Document cases according to Inspire Pakistan standards and maintain all records in a secure and confidential way.
  • Provide case management services through high quality and confidential interventions in line with minimum standards for service provision and Inspire Pakistan and UNHCR case management SOPs. This will include assessment of needs, development of an action plan, conducting referrals to relevant services, implementing the action plan and ensuring appropriate follow up and case closure when appropriate.  
  • Provide age-appropriate counselling for vulnerable through the implementation of tools developed for caring related to protections.
  • Ensure the utmost confidentiality of case management data and information throughout the case management process; including protection referrals.
  • Interviewing PSNs/PWDs to assess their situational needs.
  • Determining which services and assistance are necessary.
  • Serving as a liaison between the PSNs/PWDs and government/ community services.
  • Developing a plan of action to improve the PSNs/PWDs well-being.
  • Planning regular follow up to assess progress and additional needs of the PSNs/PWDs.
  • Reviewing and modifying plans according to circumstantial changes of the PSNs/PWDs.
  • Ensure that all case management related M&E tools are utilized, information analyzed and actions taken to ensure quality service provision.
  • With the help and guidance of case manager develop, execute and monitor case management action plans/safety/care plans for beneficiaries, developed by Case Workers;
  • Work in close coordination with Case Manager and Community-based Protection Officers on issues related to program, activities and regularly update referral pathway in collaboration with Social Welfare Department, Health department, Education Department and other service providers.
  • Participate and ensure participation in peer-to-peer or other capacity building initiatives implemented at the district level.
  • Strong verbal and written communication skills to explain to PSNs, family members, friends and professionals, the case and care plan and maintain good case records.
  • Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from CBP Officer, OVs, Health Care and social workers
  • Computer literacy specially skills of Excel, Word and other data collection software to maintain and manage case records.
  • Being familiar with process of case management and its reporting mechanism through assessment.
  • Any other duties as assigned by the supervisor.
